Reevaluation Readiness Checklist

If you worry about a proposed schedule change or a reexamination, organize your current rating package and continuity of care first.

Checklist

  • Latest rating decision / award letter: Extract SC conditions, percentages, effective dates, and any temporary or protected language.
  • 12-month treatment continuity: Recent appointments, medications, devices (CPAP, hearing aids), therapy notes for each SC condition.
  • Current functional impact: Work limits, ADLs, flare frequency, worst-day examples per condition — not just diagnosis names.
  • Private records VA may not have: Community care, sleep studies, audiology, mental-health notes, imaging.
  • Updated lay statements: Family/buddy observations that match your current severity, dated and specific.
  • If reexamined: Bring device data, medication list, symptom log, and describe residual limits honestly even when treatment helps some days.
  • Accredited help handoff: Prepare a packet for a free VSO or accredited representative before choosing supplemental, increase, or wait-and-document strategies.
